Social services - support groups: Outline
START provides an assessment service to help you regain independence with daily living activities.
We can help with:
- assessment to live independently in the community
- practical help and advice
- assistance with early and safe discharge from hospital
- help and support to avoid the need for a person to be admitted to hospital, respite care or residential care
