Targeted Services

If you are struggling to meet the needs of your child or young person and have already tried accessing support through your local Children's Centre or child's school or setting then a Common Assessment Framework (often referred to as a CAF) may help to identify extra support from other services. A CAF can be completed with you and your child by any service or setting that you already have contact with i.e. child's pre-school, nursery, school, children's centre, Health Visitor, GP etc. This will look at the circumstances surrounding your child identifying strengths and needs and with your consent be discussed with other professionals to see how they may be able to support your situation.

When completing this Common Assessment Framework you will be allocated a Lead Professional who will act as your point of contact to avoid you becoming overwhelmed with information from lots of different sources. This lead professional will normally be someone you and your child already know and feel comfortable with.
