Cremation
If the body is to be cremated and has not been referred to the coroner or, when referred, a Notification by the Coroner (Pink Form 100) has been issued you will need:
- Preliminary Application for Cremation (Pink Form)
- Registrars Certificate (Green)
- Application for Cremation (Cremation Form 1)
- Medical Forms (Cremation Forms 4 and 5)
- Authority to Cremate (Cremation Form 10)
If the body is to be cremated and a hospital post mortem has taken place and the results are known to the doctor who attended the deceased before death you will need:
- Preliminary Application for Cremation (Pink Form)
- Registrars Certificate (Green)
- Application for Cremation (Cremation Form 1)
- Medical Form (Cremation Form 4)
- Authority to Cremate (Cremation Form 10)
If the body is to be cremated and a post mortem examination has taken place and an inquest opened you will need:
- Preliminary Application for Cremation (Pink Form)
- Application for Cremation (Cremation Form 1)
- Coroners Certificate for Cremation (Cremation Form 6)
- Authority to Cremate (Cremation Form 10)
Burial
If the body is to be buried and the death has not been referred to the Coroner you will need:
- Burial Authority Interment Form
- Registrars Certificate (Green)
If the body is to be buried and there is an inquest you will need:
- Order for Interment
- Coroners Order for Burial (Form 101)
