Use this form to apply for a chaperone license so that you may supervise children during a performance. You must be over 18 and living in Southend-on-sea, Westcliff-on-sea and Leigh-on-sea or Shoeburyness to be issued a license. If you live outside of those locations, you should apply directly to Essex County Council.
You will need to upload 1 passport style photo, a form of identification and proof of address, details can be found here on what can be accepted: -ID/documents required for E-DBS checks – Southend-on-Sea City Council
If you currently hold an active DBS certificate you will need to upload this also.
At the end of the form, you will be required to pay for your license. Please have your payment details available.
